Care Management during your hospital stay

Being in the hospital can be challenging, especially when you’re not feeling well or you’re faced with complicated health issues

The PHP Care Management team will help coordinate and manage your care so that you can focus on your recovery. To help coordinate your care, support your recovery, and return home, your PHP Care Manager will:

• Monitor your daily progress
• Coordinate with your hospital physician to assure you have the right care at the right time
• Participate with you, your family, and the hospital staff to plan your discharge
• Facilitate authorizations for the services you may need when you leave the hospital
• A PHP Care Manager may also introduce to you a unique tool called a Personal Health Picture to help you organize all your medical information

When you are discharged, our goal is to help you function the very best you can. Your physician may prescribe additional services to support your recovery. Your PHP Care Manager will review these recommendations with you and your family members prior to discharge. When medically necessary, these services are usually paid for by your insurance.

Care Management following your hospital stay

The PHP Care Management team will continue to help to coordinate your care, and help support your recovery upon your return to your home. Through our Transitions in Care Program, the PHP Care Management team will contact you after your discharge to set up a home visit for additional support.
